    THE POSITION
    The California Children Services (CCS) Community Health Worker works under the general supervision of a Program Administrator and will mainly support CCS Public Health Nurse Case Managers and registered professional staff in providing community outreach, education, case coordination, provider support and assistance.

    THE PROGRAM
    The CCS program is a state program that provides diagnostic, treatment, medical case management, and physical and occupational therapy services for children and youth under age 21 with certain medical conditions. Examples of CCS-eligible conditions include, but are not limited to, chronic medical conditions such as cystic fibrosis, hemophilia, cerebral palsy, heart disease, cancer, traumatic injuries, and infectious diseases producing major sequelae. CCS also provides medical therapy services that are delivered at public schools.

    The CCS program is administered as a partnership between county health departments and the California Department of Health Care Services (DHCS).

    THE IDEAL CANDIDATE
    The ideal candidate will be compassionate with a deep sense of empathy and cultural sensitivity. They will have strong communication, organization, case management and coordination skills with a passion for serving the community. The ideal candidate has extensive experience coordinating and conducting outreach activities, knowledge of community resources, assessments, facilitating Medi-Cal referrals, coordinating specialty appointments, and clerical duties.

    EDUCATIONAL INCENTIVE: Some positions may be eligible for educational incentive. This incentive may be 2.5%, 3.5%, or 5% for incumbents in eligible positions based on completion of an Associate's, Bachelor's, or Master's degree that is not required for the classification.

    DEPARTMENT/AGENCY: Health Care Agency - Public Health

    Community Health Worker is represented by the Service Employees' International Union (SEIU) and is eligible for overtime compensation.

    The eligible list established from this recruitment may be used to fill current and future Regular (including Temporary and Fixed-term), Intermittent, and Extra Help vacancies for this position only. There is currently one (1) full-time, Regular vacancy.

    Examples Of Duties

    Duties may include but are not limited to the following:
    • Client Intake: assists in the initial client assessments and follow up including basic intake interviews; assists clients to define problems and needs, and place them in order of priority;
    • Assists the Public Health Nurse (PHN) to request information from providers to assess medical eligibility; coordinate client follow-up with medical providers; help clients make appointments and provide assistance for them to keep appointments; report findings to PHN;
    • Evaluates client's health insurance coverage needs and gaps; connect clients with eligible benefits such as State Disability SSI, SSDI, and Medi-Cal by completing the program eligibility process;
    • Keeps appropriate compliance and data keeping records that includes documenting client encounters, community outreach activities, provider outreach, and progress notes consistent with agency requirements;
    • Analyzes data and creates reports;
    • Assists in implementing coordination of care plan and monitor client compliance;
    • Provides Community Education and Outreach by creating and using standardized materials good customer service skills verbally, written and with visual aids while adapting the outreach to the targeted population;
    • Facilitates access to CCS services through community and provider outreach and conducts presentations on CCS services, program eligibility and the referral process;
    • Performs outreach activities by identifying a pool of potential paneled medical providers and informing about health services covered by CCS and the Medi-Cal programs to increase the local CCS provider pool;
    • Acts as Department liaison with community groups and clients and informs them about community services;
    • Coordinates services and collaborates with other agencies to perform a variety of activities related to program planning functions;
    • Addresses barriers to care by linking patients/clients to community resources and provides case management services;
    • Assists patients/clients to enroll in programs that cover social needs;
    • Follows program guidelines and regulations, including eligibility;
    • Meets productivity and quality goals set by agency;
    • Participates in care team huddles, meetings, and required training;
    • Performs clerical activities related to general office procedures; and
    • Performs other related duties as required.


    Typical Qualifications

    These are entrance requirements to the exam process and assure neither continuance in the process nor placement on an eligible list.
    EDUCATION, TRAINING, and EXPERIENCE:
    High school diploma, GED or equivalent, plus any combination of education and experience which has led to the acquisition of the required knowledge and abilities.
    NECESSARY SPECIAL REQUIREMENTS:
    • Must be bilingual (reading, writing and speaking) in English/Spanish
    • Experience with electronic health records
    • Experience using the Microsoft Office suite, including PowerPoint, Excel, and Word
    • One (1) year of experience facilitating presentations to large and small groups and coordinating outreach activities,
    • One (1) year experience facilitating program eligibility and care coordination services that includes linking patients/clients to appropriate community resources
    • Experience collaborating and working with a diverse population, which includes community partners and medical professionals
    • Must possess or obtain a valid California driver license by date of hire and maintain it thereafter
    DESIRED:
    • Knowledge of Medi-Cal claims
    • Experience working with either private health insurance or public insurances
    • Experience working with Federal (i.e., Social Security) and/or State (i.e., Medi-Cal) programs
    KNOWLEDGE, SKILLS, and ABILITIES:
    Some to thorough knowledge of: community service organizations that provide services to the agency’s target population; medical terminology; community agency program policies and procedures; group and family dynamics; specialized health related topics such as communicable diseases and preventative health maintenance; Medi-Cal guidelines; Medi-Cal billing; Medi-Cal eligibility, health services covered, and how to access Medi-Cal health programs; and safe driving practices.

    Skilled in: Computer literacy in a Windows environment; Microsoft Word, Microsoft Excel, and Microsoft PowerPoint; reading, writing, and speaking in Spanish; developing trusting and effective working relationships with a community partners, medical provides, and a diverse client population; leading and/or working effectively in a team environment.

    Ability to: Effectively communicate with individuals and groups from diverse cultural, socio-economic backgrounds; maintain case management notes on the state database system; monitor client compliance; public speak in front of a variety of audiences.

    WRITTEN EXAMINATION - 100%: An un-proctored, online, job-related written examination will be administered to test candidates' knowledge of language usage, interviews/confidentiality, medical knowledge and terminology, interpersonal relations, and filing. Candidates must earn a score of seventy percent (70%) or higher to qualify for placement on the eligible list.

    Candidates successfully completing the examination process may be placed on an eligible list for a period of one (1) year.
