    Description

    Under general direction, this role manages implementing the Enhanced Care Management (ECM) program for young children and families with complex medical and social needs. The role oversees ECM service delivery in alignment with CalAIM requirements, supervises staff, and ensures program quality, partner alignment, and regulatory compliance. The role co-develops clinical workflows and protocols to support implementation of the ECM benefit for child-centered and birth equity populations.

    • Oversees delivery of ECM services and ensure alignment with CalAIM requirements.
    • After ECM staff are hired, supervises and supports them to ensure high-quality, coordinated service delivery.
    • Supports the development and review of comprehensive care plans to ensure a coordinated whole-person approach for Medi-Cal members
    • Develops and refines operational workflows and processes to support effective ECM implementation and program operations.
    • Coordinates with Managed Care Plan partners to support program alignment and ensure compliance with contractual, quality, and promising practices standards.
    • Ensures accurate and timely staff documentation to support billing requirements and necessary reports.
    • Assesses team performance and communicates findings to leadership; implements changes to improve efficiency and service quality; maximizes effectiveness and ensures alignment with department’s mission
    • Recommends and implements goals and objectives, policies and procedures and ensures the program is staffed with qualified employees by supporting hiring, training, performance management, and staff development
    • Oversees daily program operations by setting expectations, communicating goals, and monitoring work to ensure quality, timeliness, and compliance with policies
    • Develops and implements program plans and resources, and provides training to ensure staff and partners understand their roles and responsibilities


    Qualifications

    Minimum Qualifications:

    Any combination of training and experience that would provide the required knowledge, skills, and abilities is qualifying. Additional experience and/or education can be substituted to meet typical qualifications. A typical way to obtain the required qualifications would be:

    5+ years of experience in care management, case management, or health/social service delivery; 3+ years of program management and supervisory, administration, or team leadership experience.

    Master’s degree in social work or a related health field.

    Licenses and Certifications

    Current and active License in one of the following is required:

    • Licensed Clinical Social Worker (LCSW)
    • Licensed Marriage and Family Therapist (LMFT)
    • Licensed Professional Clinical Counselor (LPCC)


    Knowledge of:

    • Strong understanding of CalAIM, Enhanced Care Management (ECM), Community Health Worker (CHW) Benefit, and Medic-Cal managed care operations; experience developing operational workflows.
    • Knowledge of health care and social determinants of health.
    • Knowledge in serving families experiencing homelessness, involved with child welfare, navigating serious medical, developmental or behavioral challenges.
    • Principles and practices of employee supervision, including work planning, assignment review and evaluation, discipline, and the training of staff in work procedures.
    • Principles and practices of leadership.
    • Principles and techniques for working with groups and fostering effective team interaction to ensure teamwork is conducted smoothly.
    • Principles and practices of program management including planning, development, implementation, and evaluation.
    • Modern equipment and communication tools used for work functions and program, project, and task coordination, including computers and software programs relevant to work performed.


    Ability to:

    • Ability to manage programs, implement new service models, and ensure compliance with contractual and regulatory requirements.
    • Excellent communication, facilitation, and partnership-building skills.
    • Experience collaborating with managed care plans, community partners, and cross-functional teams
    • Ability to use data for program monitoring, quality improvement and reporting.
    • Ability to guide teams through change in a fast environment; leadership, problem-solving and adaptability skills
    • Develop and implement goals, objectives, practices, policies, procedures, and work standards.
    • Provide administrative, management, and professional leadership for the assigned program.
    • Select and supervise staff, provide training and development opportunities, ensure work is performed effectively, and evaluate performance in an objective and positive manner.
    • Ensure the maintenance and retention of complex records, files, reports, and other documents.
    • Independently organize work, set priorities, meet critical deadlines, and follow-up on assignments.
    • Effectively use computer systems, software applications relevant to work performed, and modern business equipment to perform a variety of work tasks.


    Please note:

    • This position is funded through 2026, with the goal of sustaining the role beyond 2026 through Medi-Cal reimbursement.
    • We are currently working a hybrid 2 day per week in-office schedule and employees also come to the office to attend meetings and as requested.
    • All First 5 employees must live and work in California.
    • The salary range for this position is $99,000-$142,000 and as an independent government agency, the posted salary is the set salary range for this position. We cannot offer salaries above this posted range.
