    The Contra Costa Health Department is offering excellent employment opportunities for dynamic and experienced individuals that are ready to serve the community in the Mental Health Family Services Coordinator position. The department currently has six (6) vacancies in the Health, Housing & Homeless Services Division (H3) Division located in Martinez, CA.

    The Mental Health Family Services Coordinator will support housing and outreach programs by working closely with multidisciplinary teams to coordinate services, participate in case conferences, and ensure documentation and quality standards are met. They will also engage in community outreach, help track program performance, and collaborate across systems to promote housing stability, service engagement, and overall program effectiveness.

    Health, Housing & Homeless Services (H3) leads the County’s systemwide response to homelessness, including administration of the Continuum of Care (CoC) program and the County’s Homeless Information Management System (HMIS). Additionally, this program has oversight of emergency shelter programs, supportive housing, California Advancing and Innovating Medi-Cal (CalAIM) services, Coordinated Outreach, Referral, and Engagement (CORE) program, and the County’s street-based outreach initiative.

    • Coordinating services and referrals across housing, behavioral health, and social service systems to connect people to care and housing
    • Serving as a liaison with County programs and community partners to support operations and resolve service barriers
    • Overseeing day-to-day operations for assigned housing or outreach programs to ensure regulatory and funding compliance
    • Monitoring program performance and outcomes and recommends improvements to service delivery.
    • Participating in interagency coordination and community engagement to support homelessness response goals.
    • Interviewing, providing recommendations for hire, training, supervising, and evaluating staff delivering housing and outreach services

    Minimum Qualifications

    Education: Possession of a Bachelor’s Degree from an accredited college or university with a major in psychology, counseling, or a closely related field.

    Experience: Two (2) years of Full-Time experience, or its equivalent, providing advocacy services in a mental health program.

    Substitution: An additional year of Full-Time, or its equivalent, experience may be substituted for the required academic major. Additional experience providing advocacy services in a mental health program may be substituted for the required education on a year-for-year basis.

    Desirable Qualifications:
    • Experience working with homeless services, housing programs and Continuum of Care (CoC) systems
    • Knowledge of federal and state regulations for homeless funding sources such as Housing and Urban Development’s (HUD) Continuum of Care (CoC) Program, service program requirements and grant compliance standards
    • Experience with Homeless Management Information System (HMIS) and Electronic Health Record (EHR) data systems
    • Ability to coordinate cross-agency and multi-partner initiatives
    • Skilled public speaker and meeting facilitator
    • Experience working with diverse and underserved populations
    • Experienced Project Manager with program development expertise


    Selection Process

    1. Application Filing and Evaluation : Applicants will be required to complete a supplemental questionnaire at the time of application. Applications will be evaluated to determine which candidates will move forward in the next phase of the recruitment process.

    2. Online Interview Assessment : Candidates that meet the minimum qualifications will be invited to participate in an online, on-demand interview. The interview will measure candidates' competencies as they relate to the job and will be rated by subject matter experts. In the interview, candidates must achieve an average passing score of 70% or higher on each of the competencies, as well as an overall passing score of 70% or higher. These may include but are not limited to: Action & Results Focused (Taking Initiative): Involving Others (Engaging Teams), Oral Communication, Professional & Technical Expertise, and Valuing Diversity. (Weighted 100%).

    The online interview assessment will be administered remotely using a computer or mobile device such as a tablet or smartphone. You will need access to a reliable internet connection to take the assessment, as well as a computer or mobile device with a camera.

    The online interview assessment is tentatively scheduled to take place via computer (remotely) sometime during the week of April 16, 2026.
