POSITION SUMMARYThe City of Fort Lauderdale is seeking experienced, client-centered Case Managers to provide intensive street outreach and comprehensive support to individuals and families experiencing homelessness. This role is ideal for someone skilled in trauma-informed care, motivational interviewing, and working with highly vulnerable populations.
This job classification is included within the bargaining unit represented by Federation of Public Employees and therefore is subject to the terms and conditions of the Collective Bargaining Agreement between the City of Fort Lauderdale and Federation of Public Employees.
ESSENTIAL JOB FUNCTIONSEssential functions are the basic job duties that an employee must be able to perform, with or without reasonable accommodation. - Conduct proactive street outreach and provide crisis intervention as needed.
- Complete client assessments, intakes, and individualized case plans.
- Assist clients in obtaining identification, benefits, healthcare, and social services.
- Support clients with housing navigation and long-term stability planning.
- Collaborate with multidisciplinary partners, including the Fort Lauderdale Police Department Homeless Outreach Team (HOT), Fort Lauderdale Fire Rescue Mobile Integrated Healthcare (MIH) team, and Fort Lauderdale Police Department Co-Responder mental health team.
- Participate weekly in Community Court and attend meetings/trainings.
- Maintain accurate documentation and enter data into HMIS and internal tracking systems.
- Performs related work as required.

PREFERRED QUALIFICATIONS: Masters degree in Social Work, Psychology, Sociology, Human Services, or related field
As public servants, employees may be required to work immediately before, during, and/or after an emergency. PHYSICAL STANDARDS REQUIRED TO PERFORM ESSENTIAL JOB FUNCTIONS: The position is generally sedentary. Employees sit most of the time, but may walk or stand for brief periods of time.
