    POSITION SUMMARY

    The City of Fort Lauderdale is seeking experienced, client-centered Case Managers to provide intensive street outreach and comprehensive support to individuals and families experiencing homelessness. This role is ideal for someone skilled in trauma-informed care, motivational interviewing, and working with highly vulnerable populations.
    This job classification is included within the bargaining unit represented by Federation of Public Employees and therefore is subject to the terms and conditions of the Collective Bargaining Agreement between the City of Fort Lauderdale and Federation of Public Employees.

    ESSENTIAL JOB FUNCTIONS

    Essential functions are the basic job duties that an employee must be able to perform, with or without reasonable accommodation.     • Conduct proactive street outreach and provide crisis intervention as needed.
    • Complete client assessments, intakes, and individualized case plans.
    • Assist clients in obtaining identification, benefits, healthcare, and social services.
    • Support clients with housing navigation and long-term stability planning.
    • Collaborate with multidisciplinary partners, including the Fort Lauderdale Police Department Homeless Outreach Team (HOT), Fort Lauderdale Fire Rescue Mobile Integrated Healthcare (MIH) team, and Fort Lauderdale Police Department Co-Responder mental health team.
    • Participate weekly in Community Court and attend meetings/trainings.
    • Maintain accurate documentation and enter data into HMIS and internal tracking systems.
    • Performs related work as required.


    JOB REQUIREMENTS, PREFERENCES & WORK ENVIRONMENT

    MINIMUM JOB REQUIREMENTS:

    Bachelor’s degree in Social Work, Psychology, Sociology, Human Services, or related fieldMinimum 3 years of case management experience with vulnerable or unsheltered populationsStrong skills in Motivational Interviewing, Harm Reduction, and Trauma-Informed CareExcellent verbal and written communication skills Proficiency in Microsoft ExcelAbility to walk long distances and work outdoors in all weather conditionsIf claiming Veteran's Preference, candidates must attach a DD214 Member 4 form
    PREFERRED QUALIFICATIONS:

    Masters degree in Social Work, Psychology, Sociology, Human Services, or related field
    As public servants, employees may be required to work immediately before, during, and/or after an emergency. All employees must be available and able to work assigned shifts as determined by their Department Heads.

    PHYSICAL STANDARDS REQUIRED TO PERFORM ESSENTIAL JOB FUNCTIONS:

    The position is generally sedentary. Employees sit most of the time, but may walk or stand for brief periods of time. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
