    Overview

    Are you a compassionate and experienced nurse looking to make a lasting impact in the lives of children and youth with special healthcare needs? Join the Department of Health and Environment (DHAE) as a part-time, Senior Public Health Nurse!

    The Nurse Navigator will provide specialized support to children and youth with special healthcare needs (CYSHCN) and their families within the HCP program. This includes offering information and referrals, coordinating care, facilitating access to services, and helping families navigate the healthcare system. The Nurse Navigator will collaborate with healthcare providers, specialists, and community resources to ensure coordinated, comprehensive care that meets individual health needs and improves outcomes.

    This role involves working with the Maternal and Child Health (MCH) Block Grant to align with state health objectives, and contribute to strategic measures that promote health equity, community inclusion, and those which address the root causes of health disparities in Larimer County’s MCH population. This Senior level nursing position will assist in preparation and monitoring of the budget for assigned programming, monitor and support grant objectives, and provide lead accountability and supervision for other employees within the work unit.

    While the full salary range is provided in the job description, initial compensation is generally offered between the minimum and midpoint of the range, based on the candidate’s experience, education, and internal equity considerations.

    The typical schedule for this position is 32 hours per week, Monday through Friday, with a hybrid arrangement of remote, in-office, and field work during regular business hours. Occasional assignments outside of business hours may be required for outreach events.

    Responsibilities

    What you'll be doing:
    • Assess the physical, emotional, social, and environmental needs of clients and their families as they relate to health and life-course development, and develop plans to meet these needs according to program guidelines through home visits and phone contact.
    • Collaborate with medical providers, specialists, social workers, and other community organizations to create individualized care plans for each child.
    • Assist families in understanding complex medical diagnoses and treatment options, providing emotional support, and empowering families to make informed healthcare decisions.
    • Provide education, resources, and referrals as appropriate.
    • Identify gaps and barriers in community systems for clients and families.
    • Will exercise direct or indirect supervision of staff.
    • Maintain accurate and up-to-date records of patient care and interactions, ensuring compliance with program policies through state required data collection and documentation in Electronic Health Record.

    What we're looking for:
    • Have a compassionate, patient, and empathetic approach to care.
    • Experience working with children and youth with special health care needs or chronic conditions is highly desirable.
    • Prior experience in care coordination, case management, or patient advocacy is a plus.
    • Excellent communication and interpersonal skills, with the ability to interact effectively with families, healthcare providers, and community organizations.
    • Strong organizational skills with the ability to manage multiple tasks and prioritize effectively.
    • Knowledge of local, state, and federal programs that support children and families with special health care needs (e.g., Medicaid, Children's Health Insurance Program (CHIP), Early Intervention (EI).


    Qualifications

    • Bachelor's Degree, or equivalent education, in Nursing from an accredited college required.
    • Licensed to practice as a registered nurse (RN) in the State of Colorado required.
    • Three (3) years of increasingly responsible nursing experience with some responsibility for assisting in program development required.
    • Prior experience in specific nursing program relevant to the role they will perform preferred.
    • Maintain continuing education hours as required by the Department, and obtain certificates/training necessary for specific programs required.
    • Possession of CPR certification required.
    • Possession of a valid Driver's License by date of hire required.
