ABOUT THE POSITIONThe Clark County Department of Social Services is seeking qualified candidates to apply for the Senior Eligibility Worker position. This role receives client referrals with diagnosed medical conditions, mental health disorders, substance use disorders, or co-occurring disorders. Responsibilities include conducting assessments to determine eligibility for prospective programs and community services, assisting clients with document readiness for permanent supportive housing and ensuring participants maintain temporary emergency housing. The ideal candidate will possess extensive experience working with individuals experiencing homelessness or who have previously experienced homelessness, particularly those with a history of mental illness or substance use disorders. Strong interpersonal and communication skills are essential, as you will collaborate closely with program staff, community partners, and clients from diverse backgrounds. Candidates must have a demonstrated ability to manage complex cases, and promote best practices in the provision of housing supportive services. This position requires relevant professional experience in case management, housing services, or a comparable setting.

MINIMUM REQUIREMENTSEducation and Experience: Equivalent to graduation from high school AND four (4) years of full-time experience in determining eligibility for various social service, medical or related programs. Professional level experience and/or education that has provided the skills and knowledge necessary to perform all job functions at this level may be substituted on a year-for-year basis.All qualifying education and each experience must be clearly documented in the “Education” and “Work Experience” sections of the application.

Working Conditions: May be required to visit individual client residences.

EXAMPLES OF DUTIES- Provides lead direction, training and work review to a small group of Eligibility Specialists, Eligibility Workers and related support staff; organizes and assigns work, sets priorities and follows-up to ensure coordination and completion of assigned work.
- Provides input into selection decisions, performance evaluations and disciplinary matters.
- Reviews case notes and files and determines appropriate courses of action in such difficult, complex and specialty areas as nursing home placement, Medicaid coverage, hospital discharge, court casework, etc.; makes recommendations for appropriate coverage and referrals.
- Provides outreach to the community or client base to which assigned; makes clients aware of services and community resources available to them.
- Provides basic referral and casework service to clients in collaboration with professional social work staff. Interviews clients to obtain information, determine eligibility for various social service programs and assess the appropriateness of referral to other community assistance or social service agencies.
- Obtains and verifies income, asset insurance and related information; contacts employers, banks, social security, veterans' administration and other public and private sources as necessary.
- Assists clients in completing applications; provides factual information regarding various program regulations and procedures; may act as an advocate for the client with other organizations.
- Creates and maintains case files; maintains accurate documentation regarding all information received and actions taken.
- Determines the need for specific types of assistance, such as transportation, medical care, and appliances for the disabled, senior companion or burial services.
- Maintains accurate logs and complies records of work performed.
- Contributes to the efficiency and effectiveness of the unit's service to its customers by offering suggestions and directing or participating as an active member of a work team.
- May make home visits to obtain and verify case information; prepares case notes of such home visits.
- Enters client information into an automated database system.
- Uses standard office equipment in the course of the work; may drive a personal or County motor vehicle to make home visits.
PHYSICAL DEMANDSMobility to work in a typical office setting and use standard office equipment, vision to read printed materials and a computer screen, and hearing and speech to communicate in person or over the telephone. Specified positions may require mobility to make home visits.
