    Position Assignment/Duties:

    California Children’s Services PHN Nurse Case Manager provides case management and coordination of medical care for children who have CCS medically eligible conditions. The PHN determines medical eligibility, issues authorizations, makes referrals, and provides care coordination as a participant and member of the CCS interdisciplinary case management team.

    Due to changes in case management models, state funding may require this position to provide face-to-face patient care.

    Minimum Qualifications

    MINIMUM QUALIFICATIONS:

    License Required:
    (1) Valid California Motor Vehicle Operator's License. Out of state valid motor vehicle operator’s license will be accepted during the application process.
    (2) Valid license as a Registered Nurse in the State of California, AND
    (3) Valid license as a Public Health Nurse in the State of California

    Education:Possession of a Bachelor’s or Master’s degree in Nursing from an accredited college or university.

    NOTE: Incumbents are required to furnish personal transportation for which they are reimbursed by the County.

    Selection Process

    Permanent Employees (full-time, part-time, and permanent intermittent) may bid on any open permanent position in their same classification. Employees on probation in a permanent position may not bid on any position outside of their current work unit.

    Employees must have had three (3) months in their reassigned position before they may bid on another open position. Time period begins the date they begin working in the new assignment.

    Per Diem employees may bid on any open position in the merit system classification that is equivalent to their per diem classification if they are on an active eligible list for the open merit system position.
